Does amount paid for AZ 900 is valid for 5 attempts or only 1

Indeed, you may make up to 5 attempts in a year and in every retake, you must pay the $99 as it will be considered as a new appointment.

In addition, since you mentioned that your company provided you with a voucher code, I highly recommend coordinating directly with ESI support to ask options in retaking the exam with the discount since they are responsible in handling company discounts under your organization. Furthermore, you may also coordinate with your company if they can provide you with another voucher code that you can use in taking the AZ-900 exam.
