Name form on the government-issued identification?

On post New Process! Legal Name Change Request Form, web form Microsoft Certification Profile Name Change Request Form, they provide general info to change name. On certification page, it has this doc government-issued identification. On Certification Profile page, it just has 3 boxes for name: first name (*), middle initial, last name (*).

On some countries' passport, they do not display separate names: surname (family name), middle name, given name. They just write full name. At the bottom of card /passport, they will display in form surname<

Question:

1) in Certification Profile and another profile, what is the accurate name structure?

2) And how do inspector /examiner verify name through online?
