Trying to find my old MCITP

I did a few Microsoft certificates as a Diploma about 6 years ago through a college in New Zealand called AMES IT Academy.

I've recently been offered a job in the USA and they are requesting my Diploma, the only problem is, I no longer live in New Zealand and that diploma paper is long gone.

Now, I thought that I would be able to call them up and for them to resend it to me, however the school has now permanently closed so that is also a no-go.

I figured I would try and contact Microsoft directly for the certificates, however I have no ID number or any idea of what that could be as they were all sent to me by the school.

Is there any one who can help me in regards to this, maybe by giving my name and date of birth so that I may be able to get an access code to access these certificates?

To locate your MCITP certification and your learning profile, I have initiated a private message that contains the requested information needed to help locate and verify your account, where I will be needing at least three pieces of  information to match the profile. In addition, if your MCITP was taken from Prometric you may contact their Support Team. Microsoft concluded its relationship with Prometric as an exam delivery provider last December 2014.
